NOTICE OF SUSPENSION WITH CONDITIONS Case No. 05-53-GA Notice Issued: February 22, 2007 Jeffrey A. Slocombe, P 44704, Suttons Bay, Michigan, by the Attorney Discipline Board Tri-County Hearing Panel #16.

1. Suspension - 90 Days 2. Effective February 22, 2007 Based on respondent=s admissions and the exhibits presented, the hearing panel found that respondent, in one matter, notarized an affidavit which contained the purported signature of his client but was actually signed by someone other than his client. Additionally, respondent did not notarize the affidavit in the presence of his client, or the person who signed his client=s name. The panel also found that respondent stated that he had not been the subject of a reprimand, disciplinary action, or current investigation during the preceding year when renewing his malpractice insurance. However, during the first renewal period, respondent had been the subject of one request for investigation, and during the second renewal period, he had been the subject of five requests for investigation.

Respondent=s conduct was in violation of MCR 9.104(A)(1)-(4); and Michigan Rules of Professional Conduct 8.4(a)-(c).

The panel ordered that respondent=s license to practice law in Michigan be suspended for 90 days and be subject to conditions relevant to the established misconduct. Costs were assessed in the amount of $1,832.44.
