1. Reprimand 2. Effective December 13, 2005 The respondent and the Grievance Administrator filed a stipulation for a consent order of discipline, in accordance with MCR 9.115(F)(5), containing respondent=s plea of no contest to the allegations that respondent, in a civil matter, signed a former associate attorney=s name as an affiant to a proof of service without that attorney=s knowledge or consent. Additionally, respondent was alleged to have signed a notary=s signature to the proof of service without that notary=s knowledge or consent. Respondent was charged with violations of MCR 9.104(A)(1)-(3); and Michigan Rules of Professional Conduct 8.4(b)-(c).
The parties agreed that respondent should be reprimanded. Costs were assessed in the amount of $773.48.
