NOTICE OF REINSTATEMENT WITH CONDITIONS Case No. 98-21-RP Lee Thomas Hoffmann, Jr., P-24565, Rochester, Michigan, by Attorney Discipline Board Tri-County Hearing Panel #70.

1) Reinstated; 2) Effective October 30, 1998. Petitioner was suspended from the practice of law in Michigan for a period of 180 days effective May 16, 1996. His petition for reinstatement filed in accordance with MCR 9.123(B) and MCR 9.124 was granted by Tri-County Hearing Panel #70. The panel concluded that petitioner had satisfactorily established his eligibility for reinstatement in accordance with the guidelines of those court rules. On October 23, 1998, the panel issued an Order Granting Petition for Reinstatement with Conditions. Petitioner complied with the immediate requirements of that order on October 30, 1998, and his reinstatement to the practice of law is deemed to be effective on that date. The panel also ordered that petitioner be subject to several continuing conditions for a period of two years, including practicing with the assistance of a mentor; maintaining professional liability insurance; delegating the management of his law office to qualified personnel; and maintaining a multiple entry diary system. Costs were assessed in the amount of $510.89.
