NOTICE OF REVOCATION Case No. 94-109-JC Theodore S. Forman, P-43019, Birmingham, Michigan, by Attorney Discipline Board Tri-County Hearing Panel #82.

1) Revocation; 2) Effective June 17, 1994. Respondent was convicted in the United States District Court for the Eastern District of Michigan, by jury verdict, of one count of Contempt, in violation of 18 USC 401(3). In accordance with MCR 9.120(B)(1), respondent's license to practice law in Michigan was automatically suspended effective June 17, 1994, the date of the conviction.

Upon the filing of the judgment of conviction, the Attorney Discipline Board entered an order directing the respondent to show cause why a final order of discipline should not be entered. Following show cause proceedings, the panel entered an Order of Revocation effective June 17, 1994, the effective date of the automatic interim suspension.

Costs were assessed in the amount of $524.30.
