(1) Suspension; (2) For a period of 150 days; (3) Effective September 13, 1983. The hearing panel made the following findings: On December 16, 1981, an Attorney Discipline Board Order of Sus- pension became effective and Respondent was required to notify all clients in active matters within 10 days of the change of status pursuant to GCR 968 and failed to file with the Griev- ance Administrator a proof of mailing of said notice; on March 15, 1982, Respondent executed and delivered to the Michigan Supreme Court an affidavit required for automatic reinstatement alleging that Respondent had fully complied with the terms and conditions of said order of suspension; that said affidavit was incorrect in stating that Respondent had complied with the requirement of notification of clients and that Respondent knowingly executed that affidavit for the purpose of obtaining his reactivation and reinstatement to the rolls of attorneys licensed and authorized to practice law in the State of Michi- gan. The panel found violations of GCR 1963, 953(1-6)(8) and 972.1. Costs were assessed in the amount of $398.59.
